Members of the State Health Plan have the opportunity to take advantage of one FREE basic health screening per year (January-December). This free benefit is available to employees, retirees, and their covered spouses whose primary health insurance is the state health plan. Mackey Family Practice will be providing health screenings on campus Tuesday, March 21, 2023, and Wednesday, March 29, 2023.
REGISTRATION INFORMATION:
WHEN: Tuesday, March 21, 2023, OR Wednesday, March 29, 2023 Appointment times begin at 7:00 am.

Day 1: Must register no later than March 17, 2023 Day 2: Must register no later than March 26, 2023

New Users: • Click on MFP HealthScan Login in the upper right hand corner of the screen on the home page • Register as a new patient • Complete all fields (For CompanyCode use 1016 for Day 1 and 1017 for Day 2) • Click Register • Click Sign Up for a HealthScan • Complete Survey | Returning Users: • Enter username and password • Click Sign Up for a HealthScan • Complete HealthScan survey **If you have previously signed up for a health screening last year, a new code will automatically be populated |
Website Tips for Use:
• Write down or save your username and password.
• Complete registration in one sitting and have your BCBS insurance card handy.
• It is best to register using a desktop computer rather than a mobile device.
• Each individual user must have his/her own unique email address (Couples cannot share email.)

• Chemistry Profile (BUN, creatinine, liver test, glucose and electrolytes)
• Lipid Panel (total cholesterol, HDL, LDL and triglycerides)
• CBC (red and white blood cells, hemoglobin, hematocrit and platelets)
• Height, Weight, and Blood Pressure
• Education and Health recommendations

Please do not eat or drink anything other than water or necessary medications for 12 hours prior to your screening.
ELIGIBILITY AND COST:
• Employees, retirees and their covered spouse whose PRIMARY insurance is the STATE HEALTH PLAN. Your state health plan policy number is required to file a claim.
• Non-eligible persons can participate at a discounted charge of $47.00
• Dependents are not eligible
PAYMENT
• Payments can be made online or at the time of service using cash (EXACT CHANGE ONLY), check or debit cards.
TO ACCESS YOUR RESULTS:
• Log onto www.mfpworkwellness.com
• Click MFP HealthScan Login in top right corner of screen
• Enter username and password you created at registration
• Click HealthScan Reports
• Click View HealthScan Report beside this year’s screening date
Your results will be available through Mackey Family Practice’s confidential secure web portal, www.mfpworkwellness.com, within 3-4 business days. You do not need an email from MFP Work Wellness in order to access your results; you can log in any time. If you have questions, email info@MFPworkwellness.com.
